What Is the Research About?

You are being invited to take part in a research study about the ties between health and wellness and your face. If you take part in this study, you will be one of about 1,000 people to do so.

Who Is Doing the Study?

The person in charge of this study is Dr. Karl Ricanek Jr (PI) of the University of North Carolina Wilmington. UNCW adjunct professor, Ms. Amrutha Sethuram, will be gathering and analyzing the information for the study. There may be other people on the research team assisting at different times during the study.

Do Any of the Researchers Stand to Gain Financially or Personally from This Research?

This research is being partially funded by Lapetus Solutions Inc. None of the researchers participating in this study stand to gain financially or personally.

What Is the Purpose of This Study?

The researchers conducting this project believe that certain attributes about your wellness and health can be obtained from your face. This research project and data collection will try to answer this question.

Where Is The Study Going To Take Place And How Long Will It Last?

The research procedures will be conducted at CIS building on the campus of UNCW and at the CIE building located at 803 South College Road, Wilmington NC 28403. You will need to come to either location to participate in this study. The visit will take about 30 minutes. If you choose to participate in any follow-up sessions, each succeeding visit will take about 30 minutes each. In addition, booths may be set-up around campus for purposes of data collection. All data collection will be completed in a private room/ closed booth for each subject. Booths for the actual data collection will be contained by physical walls as in a tent. The participant will not be seen by others walking in the vicinity. No subject will provide any data or photographs in open view or within listening range of any other study participant.

What Will I Be Asked to Do?

1.Complete survey questionnaire: all subjects will be asked to complete a background survey to include information on current health, diseases, and wellness. They will also be asked about cigarette smoking and alcohol use. The subject will be guided through the questionnaire by a research affiliate.

2.Ground Truth Data: subjects will be measured for height and weight and blood pressure. Blood pressure will be measured using a commercial quality automated blood pressure device.

3.Photograph/Scan: subject will be asked to sit in a fixed chair for photographs of the face. Up to 6 photographs of the face may be taken with standard visible cameras as well as near-infrared camera. A 3D face scan may be used to capture a 3D image of the face.

All digital photographs and face scans will be secured on computer that will use encryption to ensure that no one without the required access can view/edit the material.

What Are the Possible Risks and Discomforts?

To the best of our knowledge, the things you will be doing have no more risk of harm than you would experience in everyday life.

Will I Benefit from Taking Part in This Study?

You will not get any personal benefit from taking part in this study.

Do I Have To Take Part In This Study?

If you decide to take part in the study, it should be because you really want to volunteer. There will be no penalty and you will not lose any benefits or rights you would normally have if you choose not to volunteer. No one on the research team will behave any differently toward you if you choose not to participate in the study. You can stop at any time during the study and still keep the benefits and rights you had before volunteering. If you decide to have your data and photos/scans removed from the study after the data collection is completed, you will have up to 14 days to withdraw from the study. What Will It Cost Me To Participate?

There are no costs associated with taking part in this study.

Who Will See the Information I Give?

Your information will be combined with information from other people taking part in the study. When we write up the study to share it with other researchers, we will write about the combined information. You will not be identified in any published or presented materials unless you have specifically given us permission to identify you.

We will make every effort to prevent anyone who is not on the research team from knowing that you gave us information or what that information is. Names will not be associated with your data or photos/scans. A number will be given to your data and photo/scans in order to link them. Only the PI will know which number relates to which name.

However, there are some circumstances in which we may have to show your information to other people. We may be required to show information that identifies you to people who need to be sure that we have done the research correctly, such as the UNCW Institutional Review Board.

Can My Taking Part in The Study End Early?

If you decide to take part in the study you still have the right to decide if you no longer want to continue. There will be no penalty and no loss of benefits or rights if you stop participating in the study. No one on the research team will behave any differently to you if you decide to stop participating in the study. If you decide after the collection is completed, to have your data and photos/scans removed from the study, you will have up to 14 days to withdraw from the study. I understand that my participation in this research study is entirely voluntary. I may refuse to participate without penalty or loss of benefits. I may also stop participating at any time without penalty or loss of benefits. I have received a copy of this consent form to take home with me.
